FEES

CLINIC RATES
Fine Motor Assessment: Includes: Assessment, Summary Report, Review of results with parent(s)	$150.00
Individual Treatment: 45 minute session	$75.00

HOME VISITING RATES
Fine Motor Assessment: Includes: Assessment, Summary Report, Review of results with parent(s) and travel time.	$200.00
Individual Treatment: Includes 45 minute session and travel time.	$100.00
**An initial block of 8 sessions is recommended for individual treatment.

Some extended health care insurance plans provide coverage for OT services.

Services are delivered by a registered OT and are recognized as a deductible medical expense for income tax purposes.

New Students Registration and Pre-Class Screen (one time fee)	$30.00

PAYMENT AND REGISTRATION POLICIES

Individual Treatment Session

• Cancellations: Please contact your Occupational Therapist by 9 am the day of your scheduled appointment; otherwise, a $30 cancellation fee will be applied.
• Payment: payment is accepted on a "pay as you go" basis or four in advance.

Group Registration Policies

• Registrations are received on a first come first serve basis. They can be submitted by e-mail or mail.Additional registration forms can be downloaded from our website at www.handskillsforchildren.com
• Class Size: Minimum 2 students; Maximum 6 students.
• Cheques will not be cashed until classes are confirmed.
• We reserve the right to cancel a group secondary to low enrollment. Registrants will be notified one week before the first session. Any applicable cheques will be returned to the client via registered mail.
• In order to ensure appropriate placement of your child in a group, new registrants must attend a pre-class screening.
• A $50.00 one time registration fee applies; this supports administration time and pre-class screen.
• Group fees include workbooks and materials.
