Referral form

Parents are able to call us, e-mail us, or complete our online registration form directly for any occupational therapy program or service at

Hand Skills for Children. A physician's referral is not required.

In order to facilitate referrals for families, a physician or health professional can complete our referral form and fax it directly to our office.

We shall contact the family within 2 business days of receipt.  For a copy of our referral form please click here.
